There are many car rental facilities available, they are within a 3-5 mile range of the pier; and they will provide shuttles to get you back and fourth to the ship.

The port of Miami does have parking garages availabe for guests. $ 20.00 per day. Vehicles longer than 20 feet: An additional $20 per day

Parking is available at the port of Miami and it costs $20 per day. There are other public garages outside the port that are cheaper. You will have to check online to find them.

Yes, breakfast, lunch & dinner is included in the fare of the cruise. If you have dinner at a specialty restaurant, there is a cover charge but dinner at the two main dining rooms are included in the cost of your fare.

If the beds are apart when you board simply ask the steward to put them together (or leave him a note).

The beds are full sized beds that are pushed together to make a queen sized bed. Every time we have traveled (Mr & Mrs), they have had the beds already combined with queen sized bed sheets and blankets. If the bed hasn't been combined, you can ask the cabin steward to push the beds into a queen sized bed. They will bring the queen sized blankets to fit the bed.
I'm not sure if you can request it before your cruise but if you want you could call NCL's reservation desk and check with them. I would suggest waiting till you are onboard the ship. It doesn't take the staff long to rearrange the stateroom.

How can i get my ticket, if i'm a foreign visitor?

I'm a foreign visitor and I'll not in America until the Cruise vacation. How can I get my ticket as my boarding pass? Could I get on board without the ticket and what documents should I show at that time? Thanks for your answer in advance. :)

Typically they are delivered electronically to your email account and you would print them out and bring them with you.

On NCL's website there is a link for "Log into MyNCL". If you haven't already created an account, you will need to create a account using your name and reservation number.
Once you log in, you can complete the check-in process by entering the passenger details along with your passport and visa details.
2 months before your sail date, you will see the option to download the e-docs. Click on the link and you will be able to print your documents.
Even if you don't pre-fill the e-docs online, you can do it at the port but prefilling it will save time during check-in.

In general the answer would be no. However Bahamas may require visa for visitors for certain countries. I would suggest checking the consulate website for Bahamas and check if they need visas.
