Question

I hear there is an NCL shuttle service.. Is it available from a New Jersey hotel and at what cost ?

There are none listed, as per the link below, for getting to the Manhattan Cruise Terminal from New Jersey. You could try calling NCL, number in the link below also, to see if there are any options. Depending on the New Jersey hotel location it could possibly be a relatively short taxi ride. Check with the hotel and see if they have any recommendations.

There is no real "adults only" area on the ship. Spice H2O doesn't allow kids during the day but they do allow kids at night for the parties. The Vibe is also an adults only relaxation area with lounge chairs and hot tubs.

There is no charge to use Spice H2O. For the Vibe, you have to purchase day pass.

Yes. The inside stateroom has a bed attached to the wall since it accommodates 3 guests.

Which stateroom?
If you are referring to a mini-suite, then it is not considered as a suite and you won't have the perks of a suite.
Here are the details of the perks of a mini-suite

Ebola? Really? There are other contagious diseases that are more infectious and equally dangerous.
The ships are equipped to take care of infectious diseases and if they do suspect guests of having contagious diseases they will be quarantined and provided with appropriate medical care until they can be transferred to a hospital.

All NCL ships have Wi-Fi.
You can purchase minutes in blocks of 30/60/100/250 or pay as you use.
Remember that the ship uses satellite communication for internet connection and it's quite slow.

Wifi is not free, is a satellite connection which can be slow and reception on the ship varies depending on your location on the ship.

Question

Can I get another room on the ship?

You will need to contact your travel agent or NCL sales rep directly to make the reservation changes. This Q & A forum is for answering general questions about NCL and cruising.
As far as getting another room is concerned, you should be able to upgrade your room provided there are rooms left. If you or your family members have problem climbing to reach the bunk bed, I suggest talking to a sales rep and upgrading to a balcony stateroom that has a sofa bed as the third bed.

Question

Is there a formal night or formal attire required on the Breakaway?

I ask this questions as I have cruised on other cruise lines, and there is at least one formal night

You are cruising Freestyle. Formal clothes are not required on NCL. You can wear "smart casual" clothes to the specialty dining rooms and the main dining rooms.
There are no formal nights but they do have "dress up or not" nights where formals are suggested (but not required).

The dress code is resort/smart casual the entire cruise. There are 1 or 2 "Formal or Not" evenings where it's optional to dress up if you want to and photographers are located usually in the Atrium for portrait photos. Refer to the following link for more info on what to pack/wear.
